Lazaro Puerto, 45, of Kendale Lakes, Fla., is a tough guy. He spent 13 years driving a concrete truck, wheeling it into position, climbing up the ladder in back, slinging the chute back and forth to distribute the wet concrete, 22 tons to a load.
All that hard work contributed to a herniated disc in his neck, a debilitating condition that resulted in crippling pain.
On Aug. 2, Puerto became one of the first dozen patients in the country to receive a stainless steel replacement disc that could offer hope to hundreds of thousands of people nationwide who suffer from neck pain.
Puerto realized he had a herniated disc three months ago, when the pain in his neck got so bad he could no longer drive or even play in the yard with his daughter, Lexa Victoria, 10, and his dogs Princess and Titan.
“And I don’t take pain relievers for it, because I don’t want to damage my liver,” he said.
In a two-hour operation at Miami’s Baptist Hospital, he received the replacement disc, which is designed to ease pain and, unlike old procedures, restore a greater range of motion in his neck.
The FDA approved the Prestige Cervical Disc, by Medtronic Sofamor Danek of Memphis, on July 17. It became available August 1, and South Florida doctors got right to work with it.
The implant could replace many of the 200,000 traditional cervical operations performed each year in the United States in which a damaged disc is removed and the vertebrae above and below it are fused with bone grafts, the company said.
In the new procedure, an apparatus screwed into the vertebrae above and below the removed disc rotates on a “ball-and-trough” system to restore motion.
“It’s very slick,” said neurosurgeon Sergio Gonzalez-Arias, medical director of Baptist Hospital’s Neuroscience Center, who led Puerto’s surgery. “It makes a real difference that instead of creating fusion, we’re preserving motion.”
In the neck, the spinal cord and nerves are surrounded by bony vertebrae, which are separated from each other by discs that allow the neck to rotate and bend.
A disc is like a jelly doughnut, made of tough cartilage on the outside and softer material inside, Gonzalez-Arias said. If the disc is damaged, degraded or herniated by disease or injury, some of the “stuffing” comes out, and the disc no longer properly cushions the vertebrae. It leads to intense pain and loss of motion.
Until now, the “gold standard” operation has been to remove the damaged disc and fuse the two vertebrae together with bone grafts and titanium plates. It could stop the pain, but with loss of motion.

With the new procedure, the disc is replaced by the Prestige Cervical Disc. Attached to the vertebrae with six surgical screws, it restores the neck’s motion in all directions.
“You can bend, rotate, flex, whatever you need to do,” Gonzalez-Arias said.
The new disc is not for every patient, neurosurgeons and Medtronic officials said. It should not be used for:
* Smokers and people with diabetes, who can have problems healing.
* Patients not yet 18, whose bone structure has not fully matured, and elderly patients with fragile bones. No specific ages are precluded, said Medtronic spokesman Bert Kelly, but prospective patients should consult their doctors about bone strength.
* Patients who lack mobility because of arthritis or other reasons.
* Patients whose injuries involve several cervical discs, since the artificial disc must be attached to intact vertebrae.
The FDA, in approving the disc, classifies it as a “high-risk” operation, since it involves surgery so close to the spinal cord, Kelly said. The procedure costs $32,000 to $34,000, about the same as traditional spinal fusion, and some insurance companies are paying for it, he said.
In the two-year clinical trial at 37 hospitals and medical centers that lead to FDA approval, Medtronic inserted the artificial disc in 276 patients selected randomly and performed traditional bone fusion in 265 others. It said the patients who got the artificial disc experienced less neck and arm pain, saw range of motion restored faster and were able to return to work and other activities sooner.
The FDA will require Medtronic to do a seven-year follow-up, reporting yearly.
Neurosurgeons at hospitals not involved in developing the disc expressed general approval of it.
“This is a good procedure, a moderate step forward in treating diseases of the cervical spine,” said Dr. Eric Nottmeier, neurosurgeon and assistant professor of neurosurgery at the Mayo Clinic in Jacksonville, Fla.
Mayo is training its doctors now to use the procedure, Nottmeier said. He estimated the artificial disc could be used in 30 percent to 40 percent of cases previously dealt with by disc fusion.
Medtronic spokesman Kelly said patients in the company’s clinical trials typically got back the range of motion they had before the surgery — not the greater motion of their youth. Most were able to return to old levels of activity — even downhill skiing or truck driving.
Puerto, on the eve of the operation, said he hopes to take advantage of that — up to a point. He has given up the concrete truck. He now hauls groceries for Wal-Mart.

How adept is the human frame at coping with pain? Pain is a natural function of the body and though we may not like pain the normal process of our physiology is to let us know that something is not quite right.

Researchers have found that recreational exercise has no effect on the risk of developing knee osteoarthritis (OA). The results of their study, which monitored 1,279 older adults over a 9-year study period, suggested that older adults—even people who are overweight—can participate in recreational exercise without worrying that they might develop knee OA as a result. The research was partly supported by the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS).
David T. Felson, M.D., and his colleagues at Boston University School of Medicine and Brigham and Women’s Hospital studied 1,279 adults whose parents or in-laws participated in the original Framingham Heart Study. The average age of participants at the start of the study was 53.2 years. After baseline evaluation between 1993 and 1994, the researchers followed the participants for nine years on average, to see if recreational activity levels influenced development of knee OA.
When enrolling the subjects, the researchers asked them about knee pain symptoms and activities levels (including walking or jogging for exercise and working up a sweat). The researchers also weighed the participants and examined X-rays of their knee joints to check for narrowing of joint spaces, which is a sign of OA. Approximately seven years later, researchers quizzed participants about the extent to which they exercised regularly. Two years later, they called the subjects back, weighed them again, and re-examined them for OA using knee radiographs and asking questions about knee pain and injury.

Using data from baseline and follow-up examinations, the researchers assessed whether exercise had any influence on knee OA. Analysis showed that neither recreational walking, jogging, frequently working up a sweat, nor high activity levels in comparison to peers changed the risks of developing knee OA. Comparing radiographs taken at baseline and at study completion, the authors reported that physical activity did not increase or decrease chances of narrowing of knee joint space, a typical sign of knee OA.
Being overweight or obese is a risk factor for developing knee OA. Therefore, the researchers looked into the relationship between exercise and knee OA among participants who were overweight or obese. The results for overweight or obese participants were similar to those for all participants: none of the activities that were monitored changed risks of developing knee OA or having narrowed knee joint space. That means that for obese or overweight persons, exercise does not increase the risk of developing knee arthritis, in contrast to results from previous studies.
OA is the most common type of arthritis, and is seen especially among older people. An estimated 12.1 percent of the U.S. population (nearly 21 million Americans) age 25 and older have OA. It is also more likely to occur in people who are overweight and in those with jobs that stress particular joints. Osteoarthritis mostly affects cartilage, the hard but slippery tissue that covers the ends of bones where they meet to form a joint. Osteoarthritis most often occurs in the hands, spine, knees and hips. People with osteoarthritis usually have joint pain and some movement limitations. Fortunately, most people with osteoarthritis live active, productive lives despite these limitations. They do so by using treatment strategies such as exercise, weight control, rest and relief from stress on joints, pain relief techniques, medications, surgery, and complementary and alternative therapies.

0ne day, painkillers could be colour-coded for each sex. That’s what University of McGill psychology professor Jeffery Mogil thinks, based on new research into the different ways males and females process pain.
Mogil is co-chair for the International Society for the Study of Pain. The popular assumption is that women tolerate pain better than men because of their experience with childbirth and menstruation. But among scientists, Mogil says, “The consensus now is females are a little more sensitive to pain than men.”

What is Botox?
You may already be familiar with the term ‘tetanus’ if you have ever received a tetanus vaccination. Based on a recent large-scale study, Botox is cited as an effective treatment for headaches and migraines. More than three quarters of patients in the study said that following Botox treatment, headache pain was reduced in frequency, intensity or both. The study involved patients who suffered from migraines as well as generalized head pain. Many of the subjects had tried other treatments, and these had not been successful. More than half had also stated that medications were being overused. With medication overuse causing rebound headaches, the quest for an effective headache treatment that maintains efficacy and does not cause withdrawal headaches is important for sufferers.
How is Botox Taken?
A doctor will inject Botox under an individual’s skin and into the muscle. Botox is given in a liquid form and a person will typically receive anywhere from ten to twenty-five injections to the head, neck and shoulder area. Treatments are not usually painful to receive and feel like a sharp prick.

The mechanism of head pain relief is not entirely understood. In a migraine, it is thought that Botox stops the proteins involved in pain transmission to the brain. This means that the nerves are not as sensitive to pain.

University of Manchester researchers said their finding could lead to the next generation of analgesics designed for such chronic conditions as migraines and backache.
The scientists examined microscopic amoeboid organisms commonly called slime moulds in a bid to gain greater insight into pain molecules known as ‘P2X receptors’.
“In humans, P2X receptors look identical to one another and so scientists have had difficulty understanding how they function,” said Chris Thompson, who carried out the research with Professor Alan North and Sam Fountain.

Thompson and colleagues discovered there was only a 10 percent similarity between human P2X and the slime mould equivalent. They were therefore able to deduce from evolutionary theory that it was these similar parts of the molecule that probably regulate pain in humans.
“Inhibiting P2X as a potential pain-relief therapy would be the Holy Grail of rational drug design and could revolutionize the way we manage chronic pain conditions like back pain and migraine,” said Thompson.

Scientists believe they may have identified the parts of a molecule which regulate pain in humans.
A study appearing in today’s edition of the Nature journal by researchers from the University of Manchester says chronic pain, which is usually dealt with through pain management medication, could be tackled more effectively now the structure of the P2X molecule is known.
The scientists compared a primitive P2X molecule in a microscopic amoeboid organism known as a slime mould with the human version of the molecule, establishing that only ten per cent was common between the two.

Using evolutionary theory, they deduced that these similar parts contained the parts of the molecule which regulate pain in humans.
